Where the material is on WayVote — a link is best.
Your name, postal address, phone number and email.
A statement that you believe in good faith the use is not authorized by the owner, its agent or the law.
A statement, under penalty of perjury, that your notice is accurate and you are the owner or authorized to act for the owner.
Your physical or electronic signature.

If your material was removed

If you believe it was removed by mistake, you can send a counter-notice to the same agent.
